To start your admission process, you’ll need to get in touch with Ontario Health atHome. If you are in the hospital, speak with the Discharge Planner.
These groups ensure all criteria have been met before referring you to the John M. Parrott Centre for admission. You cannot apply to the John M. Parrott Centre direcly.

To further a community that is safe, secure, respectful and supportive for older adults to live in and enjoy independence free from abuse, this resource pulls together services that people can turn to when abuse or neglect is suspected.
Information Guide to Deaths in Long-Term Care
The Public Inquiry into the Safety and Security of Residents in the Long-Term Care Homes System (LTCPI) issued 91 recommendations. In accordance with the recommendations, the Office of the Chief Coroner (OCC) has prepared an information guide (English and French versions) outlining the death reporting and investigation process in long-term care homes, including when an autopsy or inquest may be required and key OCC contacts.
